The READY BOOK™ was designed by the Vulnerable Populations Committee of Jackson and Josephine Counties, Oregon.
It was developed to deliver correct patient medical information in levels of priority. Meaning, it organizes a patient's vital medical information into stages to easily facilitate diagnosis and treatment.

INSTRUCTIONS & YOUR PHOTO

The first page of the READY BOOK™ begins with directions on how to use the book and add the necessary personal and medical information.
There is a plastic pouch that can store a photograph of you, your service animal and/or your pets.
The name of your service animal or pet can also be written on the page.

EMERGENCY PHONE NUMBERS
|
|
This is a page where you can enter all the important phone numbers in your life, from your family and friends, to your caregiver, doctor, and pharmacist.
There is also a flexible, plastic pouch for you to store important business cards, and other local numbers.

YOUR MEDICINE
|
|
There are ten (10) white pages in the READY BOOK™ for you to list all your current medications.
There is also a large, plastic pouch that can hold the current prescription printouts from your pharmacist and a place to list your allergies too.
We have found that in a medical emergency or natural disaster, health care workers need to be aware of your entire history with certain drugs, so that proper medical treatment can be given.

YOUR IMPORTANT PAPERS

In the event of a natural disaster or other emergency, important documents can be easily lost or destroyed.
This READY BOOK™ page contains a large, plastic pouch that can store photocopies of all your vital paperwork.
Copies of pertinent medical records, identification, and other important items can be stored here.

EMERGENCY SUPPLY "GO-KIT"
|
|
In case there is an emergency and you must leave your residence, the READY BOOK™ will help you design your own personal "GO-KIT."
The information and list contained in this section will help you gather together all the personal items that you'll need before you are moved to a shelter or emergency aid station.
For example, a personal "GO-KIT" might include spare eyeglasses, a hearing aid, batteries, and other necessary medical supplies.

EMERGENCY SUPPLY "STAY-KIT"
|
|
Sometimes during a natural disaster or other emergency you will be asked by authorities to stay inside your place of residence, also known as sheltering in place. If this occurs, you will need an emergency "STAY-KIT."
The information and list contained in this section of the READY BOOK™ will help you gather together all the materials that you'll need before you are asked to shelter in place.
For example, a personal "STAY-KIT" might include bottled water, blankets, and a flashlight.

YOUR I.D. BANDS
|
|
This section of the READY BOOK™ contains removable wristbands with the same barcode as your book.
Wearing the READY BOOK™ wristband will ensure that you are linked to your medical information inside the READY BOOK™.
Duplicate wristbands can also be attached to your personal items, such as luggage, your pet's collar, and medical equipment.

DISASTER SHELTER INTAKE FORMS
|
|
The READY BOOK™ pages in this section are inserted for use by health care professionals only.
These NCR forms will be used by intake personnel who will be assisting you, if you are moved into an emergency shelter facility.
The forms are provided by the American Red Cross and the Department of Health and Human Services.
